Is it filled in already?

Partly, and only the part that should be. The hazards, who is harmed and how, and the controls a good operator already runs are written for you. Likelihood, severity, the rating, your further actions, owners and dates are blank, because those are the judgements the law asks you to make about your own building.

Do I need to be a health and safety professional to use it?

No. It is written so an owner, office manager or supervisor can work through it. Every hazard is one page: read it, tick the controls you already have, cross out the ones you do not, circle a likelihood and a severity, multiply them, and write what you are going to do about anything that comes out medium or high.
